They're caused by the same problem in that Jagex pretty much remade the entire world map at some point in the past and I've not had time to remake the entire map. I was planning on doing so over the summer, although P2P icons aren't visible on the map when on F2P any more, so any Farming, Agility, D&D, etc icons would be missing.

Some of the problems with odd alignment in large dungeons are to do with how Jagex has made things work - look at Karamja Underground and you will see that the Agility Arena is
huge. Where possible, I've attempted to make things line up as well as possible, although looking at it now I can see that the Wizard's Tower isn't in quite the right place.
With the Underground Pass, the map was pretty much a copypaste from the one in the guide... is the key helpful and worth adding to other maps, or just confusing?

With that, I can see two ways to do it... either give each dungeon a title and make it clickable (imagine the Underground Pass title was a link to the guide)... or have the dungeon itself as a link, with icons linking to their respective places (see Ourania Altar as an example of this). The problem with the second option is that it would take
forever to get working well and be a pain to update - unless anybody knows of any software / Paint.NET addons that will give you a list of coordinates for the outline of an image on a transparent background (the dungeons).

Because they do go under rivers?

The one that I like is the Fremennik Slayer Dungeon - you enter the cave heading east, but the dungeon goes west.

Unless I'm mistaken, that update happened with the addition of Dungeon Maps... it's hiding Icons that hasn't been implemented... and the ability to see P2P icons on a F2P world has been totally removed.

Although I think it's pretty obsoulete with the official runescape map and dungeons but thanks none the less.

The problem with the in-game map is that it covers up the game screen so you can't see you character at the same time as the map. This lets you have both at once, has more dungeons than the map in-game and links to guides on Sal's so you can get additional information easily.
I'll take this chance to recommend the
Grab and Drag addon for Firefox which makes getting around the map
much easier and means you can use it like you do the in-game one instead of relying on the scroll bars on your browser.
