Like the Seal of Passage for Lunar Isle, this item is a must when visiting Mos Le' Harmless until you have completed Rocking Out. If you don't have it in your inventory, you can't understand what any of the pirate folk are saying.
You can also have the Barrelchest Anchor repaired by speaking to the Pirate Smith with the book in your inventory.
